CONSENT & RELEASE: I do hereby assume full responsibility for any and all damages, injuries (including death), illnesses, or losses that me or my child(ren) or any family members may sustain or incur, if any, while attending, practicing, participating or witnessing in any program, sport or physical activity occurring in or about the Courts and Facilities - "Courts and Facilities" refers to any tennis court, gym or multi-purpose room, park, building, paved surface, or playable area in or around ACDS, Alexandria City, Fairfax County, Falls Church, Northern Virginia Public or Private Schools, Little Hunting Park, Waynewood Recreation Association, and Advantage Tennis premises or at any offsite location used for Advantage Tennis programs or activities. I hereby assume full risk, waive all claims and release and hold its instructors, or partners of said program or event, individually or otherwise, harmless for any and all claims for injuries or damages. I am fully aware and understand that Advantage Tennis and associated Courts and Facilities, do not employ or contract with any medical services, provisions for ordinary or emergency medical services in affiliation with Advantage Tennis programs and offerings. In consideration of my (or my child’s) participation in and the use of Advantage Tennis equipment and Courts and Facilities, I hereby release and covenant not to sue Advantage Tennis, or any of the Courts and Facilities afore referred, nor their owners, directors, officers, employees, representatives, agents, and lessees from any and all claims resulting from any physical injury that may occur to me (or my child) while participating in any program or event sponsored or held by Advantage Tennis. I give my child(ren)/family/myself permission to participate in physical activity such as indicated above. I have no knowledge of any physical impairment that would affect or endanger my child(ren)/me from participating in the tennis program. Upon checking "Yes", I agree that in case of accident, including infection from communicable diseases, while participating in these activities, I release Advantage Tennis LLC, the hosting location, representatives, and other facilities and equipment that may be used for these tennis activities from any liability.
